What information is provided

• Name, license number, license status, license issue date, license expiration date, medical school, (medical school attendance dates coming soon).
• ACSC/QACSC (controlled substances certificate) number, status, date issued, date of expiration.
• CRNPs/CNMs in collaborative practice with and PAs/AAs registered to physicians.
• Disciplinary and public actions are provided in the bottom left portion of the license results page. A licensee with no public actions will have "No disciplinary documents were found" in the bottom left portion. License and registration actions are a permanent record. 

What information is not provided

• Some information is confidential pursuant to state law:
  • Patient complaints.
  • Malpractice case information.
  • Investigations.
• Full addresses and birth dates are not provided for security reasons.
