- Disciplinary and public actions are provided in the bottom left portion of the license results page. A licensee with no public actions will have "No disciplinary documents were found" in the bottom left portion. License and registration actions are a permanent record.

- Some information is confidential pursuant to state law:

- Investigations.
- Full addresses and birth dates are not provided for security reasons.
