Data/mailing lists are available for purchase (credit card only) in Excel format at the following rates:

$.10 per record/name for base fields (name, license number, public address) + $.01 for each additional field per record (see below for available additional fields)
$100.00 minimum.
5% credit card processing fee.

After payment is processed, you will be able to download an Excel file containing the requested data.

MD – medical doctor – full unrestricted, conditional, Interstate Medical Licensure Compact
DO – doctor of osteopathy – full unrestricted, conditional, Interstate Medical Licensure Compact
L – MD or limited or DO Without Examination – limited to program or facility (residents, fellows, professors, state mental and penal institutions
SP – special purpose (practice across state lines/telemedicine)
RSV – retired senior volunteer license
PA – physician assistant (if requested with physician license type it will be included on a separate worksheet)
AA – anesthesiologist assistant (if requested with physician license type it will be included on a separate worksheet)
TA – temporary physician assistant (if requested with physician license type it will be included on a separate worksheet)
QACSC – Qualified Alabama Controlled Substances Certificate (CRNP/CNM Only); for PAs choose PA license type and QACSC in additional field section (if requested with physician license type it will be included on a separate worksheet)
